This letter must be printed and handed to the patient at time of referral. It is important that the patient is given this letter at point of referral giving details on how to make an appointment. There is a risk your patient may not be seen in a timely manner otherwise.
Please ensure that all Physiotherapy referrals are sent using the referral template to shft.physioreferrals@nhs.net – one referral per email please.
To practices referring to Solent MSK Physiotherapy Service
From 1st September 2022 Solent MSK Physiotherapy Service will be accepting self-referrals for patients registered with South West Hampshire practices that refer to Solent Physiotherapy Service. Between now and the self-referral go live date please continue to refer patients as you do now. You will still be able to refer patients for physiotherapy after the 1st September.
