All dermatological issues (excluding 2ww) are referred to our Community Provider – Omnes – via eRS. They will triage the referral and assign it to the appropriate clinic whether this is within their community service, tor an acute provider.
- The 2ww skin pathway remains unchanged.
- Primary Care would be expected to try first line treatments before referring into the community service.
- On your referrals please attach a photo(s) and / or ensure you state the size, location and recent changes of any lesion/s that requires attention.

The following are suitable for referral:
- Mild to moderate rashes which can be managed without referral to a consultant dermatologist:
- this will include eczema (atopic and contact), psoriasis, rosacea and acne (special criteria applies, see acne section), other skin conditions such as alopecia, urticaria, nail dystrophy and skin ulcers, as well as the assessment of solitary skin lesions including benign tumours. However, the referrer must ensure that the Individual Funding Request Policy is followed.
- A range of clinical skin interventions, including skin surgery and liquid nitrogen cryotherapy.
- Skin surgical work with the exclusion of all cases that should be referred directly to Secondary Care including Suspected Skin Cancers (except low risk BCCs ) which should be referred to an appropriate Secondary Care provider under the fast track pathway (see urgent cancer referral guidelines as necessary).
Low-risk basal cell carcinomas (BCCs) will be diagnosed, treated and followed up by the community service. Medical professionals within the service will be part of the Local Specialist Multi-Disciplinary Team (LSMDT) meetings with local Secondary Care providers and meet all proper requirements of the NICE Improved Outcomes Guidance for Skin Cancer.
