There are 4 providers for multidisciplinary foot care clinics, covering 2 pathways.
Pathway 1 (UHS and Solent)
For practices in Eastleigh Southern Parishes, Totton and Waterside, and West New Forest.
Patients with a new diabetic foot ulcer should be referred urgently by letter or telephone to the NHS Solent single point of access. They will then be triaged by the podiatry service, arrangements made for immediate management, and appointments in the relevant MDFCC made.
Pathway 2 (HHFT and Southern Health)
For practices in Eastleigh, Test Valley South and Mid Hampshire.
Patients with a new diabetic foot ulcer should be referred urgently via ERS to the MDFCC at Winchester
Salisbury: They will take diabetic ulcers directly from local practices (eg Fordingbridge) or through Solent pathway.
