Locality: Mid Hampshire, North Hampshire, South East, South West, Southampton
Type of Care:
Medical photography is available with an approval from an Individual Funding Request (IFR) at Basingstoke and North Hampshire Hospital and Queen Alexandra Hospital. Only under exceptional circumstances can it be carried out at Royal Hampshire County Hospital in Winchester. There is no service at Southampton General Hospital or Salisbury District Hospital.
Please note that an Individual Funding Request (IFR) must be submitted and approved before referral.

- Patient details and contact information
- Referral letter
The service will contact the patient for an appointment by mobile phone where possible.
Once the medical photography is completed, the service then sends the photographs to the funding team as long as they have had the confirmation that the IFR has been submitted.
