MacMillian Unit Specialist Palliative Care (Bournemouth)

The Bournemouth and Christchurch specialist palliative care service accepts referrals from doctors, nurses and other health and social care professionals. Self-referrals from patients are also accepted as agreed in the East Dorset Specialist Palliative Care Review (2011); further information will usually be sought from the clinician with primary clinical responsibility for the patient (the GP for patients in the community).

Specialist palliative care advice is available 24 hours, 7 days a week on 01202 705 470.

The patient is over 18 years of age;

The patient has an advanced, progressive, potentially life-limiting illness, or there is considerable uncertainty regarding their potential for recovery (such as awaiting transplant) and the focus of care is shifting towards improving their quality of life;

The patient’s care needs cannot be met by the patient’s own primary health care team or hospital team;

The patient has complex needs in one or more of the following domains:

  • symptom control;
  • psychological support;
  • nursing care;
  • carer support;
  • complex social issues that reflect upon the patients’ physical or psychological state;

And the patient is either:

  • An in-patient at The Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust, or
  • Living in the Bournemouth, Christchurch areas and environs (East Dorset, West New Forest, Avon Valley) and is registered with a local GP. The GP may be in Dorset CCG as far north as Verwood, or west as far as the boundary with the Poole Palliative Care Service, or Hampshire CCG as far east as New Milton and as far north as Ringwood, or
  • Wishes to be under the care of the Bournemouth and Christchurch service, and lives in Poole, within one mile of the Poole/Bournemouth boundary.

All referrals must include the following information, as a minimum:

  • patient’s name, date of birth, home address and telephone number;
  • diagnosis;
  • current location of the patient;
  • reason(s) for referral.

All referrals will be assessed and prioritised, and acted upon within 3 working days of receipt by the team.

There is movement of patients between all parts of the service, without further formal referral, for example patients may be referred by the Hospital Palliative Care Team to the Community Palliative Care Team.

Patients whose disease is stable and inactive and who are facing long term disability rather than death are not normally eligible for referral to the service, but can be discussed with a senior member of the team.

All patients should be aware of the referral to specialist palliative care, except in the case of patients who lack capacity to understand or to consent to, the referral.

Referred patients may be in their own home, staying with a friend or relative, in a residential or nursing home, or in the acute hospital
