Prior approval is not required for an assessment nor is it required for the removal of a ruptured, capsulated, leaking implant (clinical need being evidenced within patient notes). It is the replacement that would require prior approval, which would only be considered on the grounds of exceptionality and of a clinical health need via an IFR application. We would accept the application from either primary or secondary care but would expect supporting information (copy of clinical letter or other) from secondary care supporting the application.
