Places available for diabetes prevention programme
News
Following difficulties we experienced last year with patients waiting for longer than we would have liked, we are pleased that Ingeus (our provider of the Healthier You programme), has successfully managed to reduce the number of patients waiting and increased course capacity significantly in 2020.
Our local NHS Diabetes Prevention Programme (NDPP), which is part of Healthier You, is reminding practices to make referrals to support people at risk of type 2 diabetes mellitus (T2DM).
The programme, available for patients at all WHCCG practices, aims to make long-standing changes to people’s health and well-being and prevent them developing type 2 diabetes. People at risk are defined by the presence of non-diabetic hyperglycaemia, and the referral requirements include:
- Fasting plasma glucose between 5.6 mmol/l and 7 mmol/l in the last 12 months
- HbA1C between 42mmol/mol and 48mmol/mol in the last 12 months
Exclusions:
- Age 17 and under.
- Previous diagnosis of diabetes.
- Pregnancy
- Terminal illness or significant frailty.
A list of available courses dates over the next two months is below.
